You have probably been told Medicare Part A is free. There is no monthly premium for most people, and that part is true. But if Part A is all you have, it comes with a $1,736 hospital deductible in 2026.
The number that decides it: 40 credits
Part A is the hospital side of Medicare. On your own work record, the number that matters is 40 credits.
You earn up to four credits a year. In 2026 that is one credit for every $1,890 you make, so $7,560 gets you all four for the year. Forty credits works out to about ten years of work, and those years do not have to be back to back.
If you have 40 of them, your Part A premium is zero. That is about 99 out of every 100 people on Medicare.
What it costs if you are short
- 30 to 39 credits — Part A runs $311 a month in 2026
- Fewer than 30 credits — $565 a month
Here is the part that stings. Medicare will not let you enrol in Part A by itself if you are paying a premium for it. You have to be enrolled in Part B too, and that is $202.90 a month in 2026 for most people.
So the real number is closer to $514 a month — or $768 if you have fewer than 30 credits.
The catch with the word “free”
Free means no monthly premium. It does not mean free healthcare.
In 2026, Part A comes with a $1,736 hospital deductible before Medicare pays anything.
Whether that bill lands on you depends on what else you have. Most Supplement plans cover it. With an Advantage plan you follow that plan’s hospital copays instead. Medicaid can pick it up. But on Original Medicare with nothing else, that deductible is yours.
And it is not once a year
You pay it once per benefit period, not once per calendar year.
A benefit period starts when you are admitted as an inpatient. It ends after you have gone 60 straight days without inpatient hospital or skilled nursing care. Go back after that, and a new deductible can start all over again.
What to do
Pull your earnings record at ssa.gov and count your credits.
And if you come up short, do not panic yet — you may be able to qualify on a spouse’s record, an ex-spouse’s record, or a deceased spouse’s record. Most people are never told that.
