Miss your 7-month Medicare window and you cannot just sign up the next month. You are locked out until January, and a penalty can follow you for life.

When the window opens and closes

Three months before your 65th birthday month, your birthday month itself, and the three months after. Seven months, and it does not move.

In that window you sign up for Part A and Part B.

The exception, and it is a real one

That is if you are retiring. If you are still working and you have solid coverage through that job, different rules apply and you can wait with no penalty at all. A lot of people qualify for this.

One catch: it is for companies with 20 or more employees. Smaller than that and Medicare usually has to be your main coverage anyway, so you cannot skip it. Check with HR before you decide to wait.

What actually happens if you miss it

This is the part nobody warns you about.

You cannot just enrol whenever you want. You have to wait for General Enrollment, January through March — and your coverage does not start until the month after you sign up. So you could sit for months with no Medicare at all.

Then the penalty. Part B adds 10% to your premium for every full year you were late, and it stays on your bill for life. Not a one-time fee. For life.

What to do

Mark the window. It opens three months before your birthday month — start there, not on your birthday.
